Abstract | Patient care is a continuum, therefore before and after heart valve procedures it is important that healthcare practitioners, patients, and carers work together to develop strategies to facilitate safe and effective transition from the clinical setting to the home environment. Understanding the diverse clinical needs of VHD patients will further enable healthcare practitioners to co-design immediate and long-term care approaches to best support patients (and their carers) through the recovery journey. |
