Abstract | MECHANISM OF ACTION Tibolone is a synthetic steroid substance that has little if any inherent estrogenic effect6. However, it is converted to two 3-OH-metabolites that bind to and activate the estrogen receptor (ER). The metabolites preferentially interact with the ERα receptor, accounting for the estrogen-like activity of tibolone on bone metabolism. In breast tissue, metabolites of tibolone inhibit proliferation of and stimulate apoptosis of breast tissue cells7. In the uterus, the peripheral metabolite of tibolone (∆4-isomer) has progestational and possibly androgenic effects. This allows tibolone to be administered without concomitant progestin in women without an intact uterus. Studies with specific inhibitors of estrogenic, progestogenic and androgenic action have documented that the effects of tibolone on bone are mediated through the estrogen receptor8. |
